CALAPH C8 Retreat

The CALAPH C8 Retreat will take place over a day and a half during which fellows will have the opportunity to participate in learning, discussing, and team-building sessions. The sessions will cover a variety of topics including health equity, agile leadership, data planning, and building partnerships.

Travel Guidance

Below you will find the CALAPH C8 Retreat Travel Guidance Document. This documents provides all details related to booking flights or trains through our travel agency, reimbursement for mileage if driving, hotel location, covered meals, per-diem, and more. Please review this document in it’s entirety prior to completing all Pre-Retreat Action Items.

Pre-Retreat Action Items

Prior to attending the retreat, each fellow is required to complete a list of pre-retreat action items. These action items will ensure smooth travel and a quality experience at the retreat. Thank you in advance for your timely attention to completing these items.

Hotel accommodations have been booked for two nights (Tuesday, January 28 and Wednesday, January 29). Details will be sent via email at a later date. Please do not make your own hotel reservations. If for some reason you are unable to stay both nights, please contact Lia Randazzo (lrandazzo@healthleadership.org) prior to Friday, December 6 for approval.

In the order listed, please complete the following action items by Friday, December 6:

    1. Carefully review the CALAPH C8 Retreat Travel Guidance Document in its entirety. This document provides all details related to booking your flights through our travel agency, driving to the retreat, hotel location, covered meals, per-diem, and more.

    2. Book your flight or train through our designated travel agency OR confirm your plans to drive or take local transportation to the retreat with Lia Randazzo (lrandazzo@healthleadership.org).

      IMPORTANT NOTE: If flying or taking a train, you must book through the CTM travel agency unless pre-approved by CHLI in advance of the reservation being made.

    3. Download and complete a W-9 form and submit directly to payments@phi.org (do not send to LAPH program staff). This form is used to process and deliver reimbursements for travel expenses after retreat. If you would like to request your reimbursements be directly deposited into your bank account, please complete a W-9 form and a direct deposit form and send both to payments@phi.org.

      • You should complete the W-9 for the person or entity that will receive the reimbursement. For example, if your agency requires the reimbursement be paid directly to them, please submit a W-9 for your agency rather than yourself. For more information on what expenses are allowable and the documentation required for reimbursement, please refer back to the CALAPH C8 Retreat Travel Guidance Document.

    • Complete the CALAPH C8 Pre-Retreat Survey. This survey will require that all steps above have been completed and will also collect information about dietary preferences/allergies and ADA accommodations.

Myers-Briggs Type Indicator (MBTI) Assessment

During the CALAPH retreat, we will be conducting a self-awareness and team building session using the well-known Myers-Briggs Type Indicator (MBTI). All CALAPH fellows will be required to take the assessment well in advance of this webinar.
