Creating healthier communities across the country
We have a proven record of helping teams address the most pressing issues in their communities to create long-lasting change by centering the needs and values of their communities. From overdose prevention to disaster preparedness and safe streets, our program gives leaders adaptable, evidence-based strategies to address a wide range of public health challenges.
Overdose prevention
We help teams build partnerships with health care providers, faith-based communities, behavioral health, law enforcement, and other stakeholders to increase patient access to treatment, reduce opioid prescriptions, and save lives.
School readiness
Our teams have launched collective impact plans to reduce inequities by race, class, and disability. Through our work with local and state leaders, children up to five years old are better prepared to enter school on a level playing field.
Food security & other chronic disease prevention
Building healthy communities includes access to fresh, healthy food. By working with leaders across the public, private, and non-profit sectors, our teams are better able to address inequity, such as working with promotoras to reach immigrant populations.
Complete streets & other environmental health
Safer communities include safer streets. Through multi-sector partnerships, our teams have made streets safer for pedestrians, increased access to public transit, and created more walkable cities.
HIV/AIDS & other communicable disease
With our support, teams across the country have developed, implemented, and evaluated tools and strategies to better reach Black, Latinx, and transgender groups of people disproportionately affected by HIV/AIDS and other disease.
Behavioral health, ACES, & trauma-informed work
We’ve helped teams launch initiatives that address domestic violence prevention, ACES and early childhood services, and trauma-informed work to make communities safer and create space for healing.
Coordinated re-entry & other housing access
By building bridges between law enforcement and community partners, our teams have developed sober living housing with wrap around support services for people leaving the county jail.
Coalition building
Creating lasting change requires effective team leadership and partnership. We support local and state leaders by giving them the tools, strategies, and coaching needed to work across sectors, engage their communities, build capacity, and ensure sustainability of larger county and state efforts.
Disaster preparedness
With our support, teams have been able to address limited availability of resources and increase volunteers prepared to respond in emergencies. From outbreaks such as Ebola, Zika, and COVID-19 to natural disasters such as earthquake, tornado, and flood, our teams are ready to mobilize quickly and save lives.
Breastfeeding & other maternal/childhood health
From access to doula care to breastfeeding to prenatal and perinatal care, we have bolstered statewide efforts to promote policies in healthcare systems that support the healthy development of families.
