Overview

The NWC Health Equity team originated from an organizational effort to become resilience trained soon after our team formed to counter the escalating COVID-19 cases: the Rapid Response team was formed to respond to this health crisis. After managing to completely flip our Belmont Cragin community from most to least affected by COVID, we continued on with the city wide effort (Healthy Chicago 2025) to close the life-expectancy gap of white folks and bipoc's in Chicago. We are currently working with the city and many other grant partners to reach a more equitable Chicago.

4 CHWs will be trained in assessing, documenting, and addressing lead and mold hazards. 100 families will receive home environmental assessments as well as receive remediation supplies. Families with identified hazards will receive letters to provide to their landlord describing conditions and requesting longer term remediation. Landlords who fail to respond within 30 days would receive follow-up from MTO.
