Overview

As a part of the Ebola Leadership Academy for the Public's Health (ELAPH), this team supports the Washington State Department of Health in partnership with Providence Sacred Heart Medical Center and Children’s Hospital in Spokane, Washington. The team seeks to improve public messaging during Ebola outbreaks/threats, as in the past, there have been communication breakdowns resulting in fear responses.

King County Partnership to Improve Community Health (PICH) Place-Based Team

Overview

The purpose of the Placed-Based project is to obtain a bigger, more sustainable impact from PICH by having a larger investment and coordinated efforts in two cities with some of King County’s largest opportunities to create healthier environments. A city was selected and included in the grant submission, and the Place-Based team is charged with providing technical assistance, utilizing a community engagement approach, and developing a process for selecting and partnering with a second city.

Overview

The Healthy Living Collaborative aims to recruit and support teams of community health workers in specifically targeted neighborhoods. They plan to form a regional network of members who practice the Community Health Workforce (CHW) skill set and provide backbone support in order to:

  1. Increase individual participation in managing health and wellness

  2. Develop stronger systemic connections

  3. Reduce emergency room usage and 'no show' rates at doctors’ visits

  4. Increase neighborhood opportunities for physical activity, healthy eating, and substance-free living

  5. Increase healthy behavior

  6. Decrease unhealthy behavior among community health workers

Overview

The proposed intervention is to create a collective impact framework that seeks to better understand and address the social determinants that lead to and result from a lack of attendance in middle school students. The team will track children after their fourth absence to better understand contributing factors and other determinants of missing school. The team will also work with the community to develop an understanding of the consequences of children missing school.
