Overview

This team will develop and implement tools that help to improve of efficiency and effectiveness of working together on the common goals of increasing childhood blood lead testing rates, improve enrollment in supportive services, and increase education and awareness among communities and healthcare practitioners.

Overview

This project will build on our previous endeavors, including close collaboration with community groups and partners in the targeted zip codes to create a community-specific strategic plan to address blood lead screening and lead hazard exposure. The Lead and Health Homes Program will provide virtual home visits, where education and home hazards assessments will be provided. Community level education will be provided for childcare staff in the target zip codes.

Overview

The team will build a comprehensive statewide, evidence-based program that meets the needs of the diverse senior population by addressing social isolation in Pennsylvania.

This project plans to build capacity to serve older adults statewide by using technology, telephone reassurance, university partnerships, and person-centered counseling.

Overview

This project focuses on increasing the consumption of fresh produce in neighborhoods through the creation of a corner store collaboration which will get fresh produce to families. The team also will be developing a toolkit that can be replicated for schools and other cities.
