Overview

As a team, we will continue to support, educate, and bring awareness to stakeholders about both the issues of maternal mortality and adverse childhood experiences (ACEs). We plan to encourage our partners to support early childhood home visiting, care coordination and access to well-woman care, prevention screening and treatment, and cultural competency training. We also plan to further educate our community collaborators on parenting, mentorship, and school-based prevention, peer norm programs, and housing improvement and programs.

Overview

Our team proposes to gain approval from local decision-makers to develop and implement a public transit system in Union County that will improve health outcomes for residents, specifically those involved in the behavioral health system. This will involve the development of formal/contractual agreements between the transit system and behavioral health agencies and social services, approval from county government to submit state and federal funding applications for the expansion of the UCATS service, gathering support and financial commitments from area businesses and major employers, and building public support for the initiative.

Healthy Community Partnership - Mahoning Valley

Overview

The project seeks to overcome a common barrier to improving health outcomes that many partners in the HCP network have encountered: a lack of trust and integrity, between service providers and clients/residents.

The team hopes to identify opportunities to move from deficit-based to asset-based language and engagement frameworks.

They will examine current methods of community engagement and language/terminology to describe the target population (Black residents, especially Black women).

They also will move towards identifying shared, asset-based language and effective strategies to work with the community on collective action to improve health outcomes.

Overview

The problem this NLAPH project seeks to improve encompasses many aspects. 72% of Van Wert County adults and 37% of 9th-12th graders are overweight or obese. Substance abuse-related fatalities have increased six-fold, bringing much concern by law enforcement, public health, and families. County rates for Medicaid transports provided by Job and Family Services is very low compared to neighboring counties.

This project seeks to improve the intersectoral collaborative necessary to positively impact the environmental, social, economic and political health of the community. The Van Wert County Health Collaborative Leadership Team proposed to put the 2017-2019 Community Health Improvement Plan (CHIP) for Van Wert County into action.

The CHIP focuses on the following three goals:

  1. To decrease the percent of obese and overweight adults and youth

  2. To increase access to evidence-based behavioral health services for youth and adults

  3. To increase access to healthcare services for those with health equities by eliminating the transportation barrier.

This will be done by: building relationships with stakeholders, increasing collaborative work across multiple sectors to achieve the goals of the CHIP, develop a framework to leverage collective resources for areas of focus in the CHIP.

Overview

The project seeks to advance and integrate the social determinants of health (SDOH) into policy and action. The team will enhance the leadership capacity of the greater coalition (12 members) by moving the agenda of SDOH integration into the agenda of influencing different sectors and disciplines. The team will advocate for the development of policies that will create the conditions for optimal health such as safe housing, adequate green space, clean air and water, access to healthy foods, access to quality health care, and quality education.

Summit County Home Delivery Model Team

Overview

The goal of this project is to use the Home Delivery Model to increase access to medications for all Summit County residents in the event of a public health emergency. The project will plan, develop, and implement operational methods to disseminate an efficient medicine dispensing system, providing prophylactic medications quickly and orderly to all county residents within 48 hours. Training and exercises will also be developed for Citizen Corps Volunteers, city and county employees, and other staff on how to effectively distribute the medication to all county residents.

Overview

The project seeks to address the misuse of prescription drugs with a focus on opiate addition, through the Opiate Task Force, working in parallel to local initiatives such as Young People Matter Coalition. This will involve exploring the relationship between the community and social norms, access and availability, and media and higher education institutions.
