Overview
This network of organizations has created a substantial health intervention foundation for the City; however, the Partnership has identified a lack of connection between individuals at risk for and victims of violence and available health intervention services.
This proposal will further the Healthier JC initiative in order to provide direct, cross-sector services to individuals suffering from violence by connecting and aligning the systems of various violence intervention service agents within Jersey City.

Overview
The North Jersey Health Collaborative will use their AHLP to identify the target population, engage partners in the development and implementation of intervention to reduce heart disease-related risk factors by 5%, and build capacity among participating organizations to increase interventions by 10%. The AHLP aims to reduce heart disease risk in asset-limited, income-constrained, and employed (ALICE) residents.

This partnership will:
Improve access to quality mental health and substance abuse prevention, treatment, and recovery services
Improve the health and well-being of the community by advocating for sustainable healthy lifestyle choices
Engage the community to prevent and reduce chronic disease incidence and morbidity (e.g., cancer, diabetes, heart disease, asthma)
Increase the overall health and well-being of county residents by enhancing safe, accessible, and affordable transportation options for people to move easily and freely within and between county communities

Don't Wait -- Vaccinate!

Overview
The Don’t Wait – Vaccinate! team will work on an applied learning project that tackles the important community health issue related to immunizations with the goal of improving community public health outcomes. Increasing immunization rates in East Orange will help protect community members from vaccine-preventable diseases including whooping cough and measles, which have caused outbreaks in New Jersey.

Overview
The project seeks to implement a new screening methodology which will incorporate the use of GIS mapping to aid in identifying high-risk neighborhoods for targeted screening and outreach. This endeavor will aid in facilitating a primary prevention approach to lead poisoning in children less than five years of age and decrease the likelihood children will suffer from negative neurobehavioral impacts of lead that can severely undermine their future.
