Overview

The team hopes to improve food security through assessment and planning with community partners. Their goal is to make food more affordable and accessible by increasing use of farmer's market incentives, increasing the reach of food shelves, and increase enrollment in WIC and SNAP programs. Also, they will connect people with resources, support breastfeeding-friendly policies, focus on underserved areas, and improve transportation equity.

Overview

The HONU Leadership Team will implement the best practices set forth by the Centers for Disease Control and Prevention for tobacco prevention. Through their AHLP, they will work toward the adoption and implementation of a tobacco free parks ordinance, the adoption and implementation of an ordinance restricting the sale and use of e-cigarettes, and the adoption of tobacco free grounds policies in the New Ulm area.

Overview

The project seeks to increase access and use of quality clinical preventative services and emergency preparedness, particularly immunizations. With their AHLP, the team aims to build stronger relationships between partner organizations, establish sustainable/reliable funding sources, cultivate new relationships within larger partner organizations, and explore ways to integrate access to other preventive health services into the project model (broaden their scope from just immunizations).

Overview

The Minnesota Collaborative Leadership Team will use their AHLP to address disproportionate rates of diabetes by:

  1. Increasing partnership engagement of 10 organizations representing African American, American Indian, or elderly communities

  2. Increasing community engagement through focus groups regarding prevention and information marketing/packaging

  3. Increasing the usage and action of marketing tools for prevention by the target community

  4. Enhancing coalition member engagement
