Overview

Their goals are to build programs, provide education, and inform policy to address the social determinants of health that contribute to health inequities for racial and ethnic minorities in Michigan.

This will include a series of webinars focused on: eliminating health inequities through educational attainment, neighborhood and built environment, health care access and quality, social and community context, and economic stability.

Community members will be engaged through various social media channels and through the team members in Detroit and Flint who are embedded and work in those communities.

Ambassadors for Preparedness

Overview

The project focuses on conducting activities to establish an effective volunteer ambassador program (aimed at disaster or emergency preparedness) by 2015. These activities include: 1) building a curriculum, policies, and procedures; 2) developing a marketing plan; 3) developing a recruitment plan; 4) creating a community toolkit for each ambassador; and 5) building sustainability plans.

Overview

The project will continue to implement the three-year strategic plan for the Lewanee Health Network which seeks to engage residents and encourage the adoption of behaviors that reduce risk factors for chronic diseases. The team will identify best-practice programs currently underway in the county, state, and nation. The project seeks to:

  • Increase healthy eating, physical activity, access to information on healthy food and exercise options, and access to health coverage

  • Decrease tobacco, alcohol, and illegal or inappropriate substance abuse use; risky sexual behavior among youth; bullying; and suicide ideation

Overview

The high rate of hypertension among this population has many underlying causes including the environment, genetics, and individual lifestyle behaviors. The project will focus on self-empowerment, individual accountability, and education through health literacy and peer-to-peer support to reduce hypertension rates. The project will have two interventions:

  • Creation and distribution of DVDs that use personal stories to promote adherence to prescribed hypertension medication instructions

  • Completion of a health literacy screening for patients and training for physicians to modify patient communications based on screenings’ results

Overview

The project focuses on built environment improvements to support behavioral changes and to serve as a model for other Michigan counties. The main activities will include:

  • Providing more health information about expanding bicycle lanes and infrastructure

  • Distributing pedometers

  • Adding support to the “StepUp” exercise program in schools

  • Expanding stair campaigns

  • Supporting urban agriculture and sustainable foods in schools

  • Promoting downtown farmers’ markets

Childhood Obesity Prevention

Overview

The project seeks to address a community epidemic of obesity by specifically targeting children 0–5 years of age in an effort to instill healthy lifestyle choices early in life by working with the tribal childcare center and referring children to nutritional counseling and support. The project also seeks to identify evidence-based policies for consideration by the tribal administration for implementation in all aspects of the community, including the school.
