Overview
They are proposing to develop a comprehensive plan for integration of behavioral health services that include developmental and social emotional screening, and connection of families to targeted interventions to prevent the manifestation of more severe physical, mental, behavioral, and academic problems. The plan will also include a family engagement component with peer to peer supports. The team will work to overcome access to evidence-based screening in early childhood for low-income, rural, and minority populations. In recent years, influential research in fields of cognitive neuroscience have brought to the attention of practitioners and policymakers alike the importance of the first years of development and the ability for children to have greater success when interventions are put into place early. Moreover, our growing understanding of the long-term effects of adverse childhood experiences, including exposure to violence and familial substance abuse, on health and well-being highlights the need for early attention to the indications of risk for such adversity.

Overview
As a part of a specialized NLAPH cohort dedicated to Ebola, the team seeks to prepare the community’s infrastructure to respond effectively to Ebola outbreaks. This community resides within an HHS-designated regional Ebola treatment center, the Georgia Department of Public Health in partnership with Emory University Hospital and Children’s Healthcare of Atlanta/Egleston Children’s Hospital in Atlanta, Georgia. The team aims to enhance the current treatment facility capacity and increase the number of treatment facilities in the local district. Furthermore, the team plans to strategize Clear and Concise Response plans, improve risk communications, and locate isolation/quarantine sites.

Overview
The goal of the project is to map community health assets and identify and address gaps, particularly their contribution to the high incidence of heart disease and strokes among the target population. The project will work specifically with two predominately African-American neighborhood clusters in Chatham County that are food deserts.

Overview
The project seeks to build the Asthma Coalition of Fulton County and play a major role in planning, coordinating, and implementing a community health needs assessment. The Fulton Asthma Improvement and Reduction program is a collaborative effort to improve the health and quality of life for those affected by asthma through implementation of evidence-based practices and systems changes. Components include:
Identifying state, county, municipal, and community-based agencies with missions that support asthma control efforts
Conducting an assessment of potential partners
Identifying an entity to convene a county collaborative
Recruiting potential partners for coalition membership
Convening a kick-off meeting
Developing an coalition organizational structure and establishing functional committees
Creating an operation plan
Establishing collaborative agreements
Developing a strategic plan
