Overview
Adverse Childhood Experiences (ACEs) affect people throughout the course of their lifespan. Studies indicate that most of the population experiences at least one ACE. However, the disease burden and all-cause mortality associated with ACEs increase concurrently with the number of ACEs that one experiences. For our leadership project, our team hopes to find new methods and avenues by which to address ACEs in the community.

Overview
The team seeks to create an environment across the county that lends itself to providing youth with STI testing resources and services that are more accessible and youth-friendly.
They hope to identify adolescent perspectives and service needs and wants through conducting student surveys.
They also will provide ongoing training to testing facilities on how to be LGBTQ+ and youth-friendly and create a checklist for community partners to use to identify and assess their level of youth friendliness.

Fast Tracking Tampa Bay: HIV Care Engagement

Overview
The team aims to identify creative strategies for case-finding and promoting and supporting engagement in care as well as key geographic "hubs" for HIV case-finding in the Tampa Bay area. Work will focus on existing and new community resources for testing people who interact within these hubs to immediately link those identified as living with HIV to care, via a supportive, personal and creative approach that provides continuity of care.

Overview
The team is working on the collaborative development and implementation of a multi-sector system that identifies and addresses the environmental and social risk factors for diabetes. The system includes screening for social determinants of health in clinical care/emergency rooms and coordinating referrals to social services. This model aims to address the underlying causes of behaviors linked to diabetes and can be facilitated by community health workers.

MDHAN: Health Access Initiative

Overview
The MDHAN team will work on the design of an innovative model to deliver cost-effective care, improve health indicators, and lower the preventable burden of chronic diseases on the community while benchmarking measures for short-term process, intermediate outcome, and longitudinal measures of impact for Miami-Dade County. The goals of this initiative are to improve overall health indicators of the community; improve patient engagement in health promotion and disease prevention; improve access to care and services; improve coordination through the continuum of care; increase employment opportunities and improve economic vitality in the community.

Overview
In consideration of Broward County's high average of people living with HIV/AIDS (PLWHA), and in response to the Affordable Care Act’s increased options for healthcare, the Organizational Development team project will seek to address medication adherence for PLWHA. In doing so, the team will work with community providers to track viral load suppression and the number of new cases of HIV & AIDS to determine if there is a decrease in the number of new cases as a result of this initiative.

Overview
The project aims to improve diabetes and cardiovascular disease health outcomes and improve access to care for the uninsured and underinsured. The project involves a better understanding of social and environmental determinants of health and furthering the development of a Community Balanced Scorecard as a tool to engage partners and manage the complex challenges in improving public health.
