San Miguel County HIA Team


Back in 2015, Yolanda Cruz and the rest of the San Miguel Team were focused on reducing recidivism among formerly incarcerated individuals to reduce poor health outcomes and socioeconomic disadvantages for the entire community. Since their time in the program, the team has been busy developing a community rehabilitation center to provide services for community members with substance use disorder and those in need of reintegration services.

Utilizing multi-sector partnership and collective impact skills and strategies gained during their time in the LAPH program, the team plans to collaborate with a local university to provide professional support to members of the program by offering services from nurses, counselors, and social workers enrolled in the university's training center. While the team initially faced challenges due to administrative changes at the county detention center, Yolanda and her team have been able to secure funding to establish an off-site rehabilitation center which will offer program participants improved access to services.
