Overview

This baseline data summary is the aggregate of information collected by our evaluation partners at the Kaiser Center for Community Health and Evaluation (CCHE). The purpose of this baseline survey was to gain an understanding of the skills, challenges, and priorities your team brought with you into the NLAPH program.
