Overview
Currently, Legacy Core is focusing on developing awareness and education around healthy cannabis consumption and use. For our leadership project, we are proposing to create communities of practice centered around cannabis use and cancer survivorship. Our goals are to increase our outreach strategies to increase consumer involvement, report an increase in customer education and satisfaction, and increase our community multi-sector participation by increasing understanding of best practices in outcome development.

Overview
The team will develop, implement, and evaluate tools and strategies for engaging heterosexual Black/African American women and gay/bisexual men in Nassau County Ending the Epidemic (ETE) programs. This includes health fairs, community outreach, community education, sexual health campaigns, provider continuing education, local and state advocacy, and ETE-related research and evaluation. Their focus is on actively seeking out community leaders outside of member organizations and using data-driven tools in efforts.

Overview
The Partnership for a Healthier New York City team will work to increase collaboration between the Department of Education, Health and Mental Hygiene, public health entities, and school stakeholders. The project also aims to: 1) increase awareness in school communities regarding resources available to support active design and built environment around schools; and 2) mobilize school communities around active design and potential for diffusion across communities.

Overview
The QTCC will focus on increasing smoke-free housing, increasing voluntary smoke-free outdoor spaces, and reducing point-of-sale marketing of tobacco products to youth living in Queens County. The QTCC will improve the process and outcomes of community organization engagement activities to develop a larger team that will support and sustain the project.

Overview
The project goal is to reduce infant mortality due to unsafe sleeping practices using an injury prevention approach that includes:
Community-based and targeted educational interventions
Engineering strategies
A partnership with the Consumer Product Safety Commission on recalled sleep aids
Enforcement strategies at local birthing hospitals for parent training, as well as with local police and medical examiners for doll reenactment training
The program will develop and implement an evidence-based safe sleep project that incorporates educational, policy, and program strategies.

Overview
The project will fully develop a capacity-building plan to address health disparities and emergency preparedness in communities with the greatest health needs and willingness and ability to participate in an organized effort to promote health and safety. The Community Health Assessment exercise will involve creating a weighted scorecard to prioritize target communities. The central goal is to develop a local action committee in each of the identified communities to work with the leadership team to improve health outcomes and emergency preparedness.
